Dog Boarding Agreement (HPI)

ABANDONMENT OF ANIMALS: If the client does not pick up or call within 3 days after the agreed upon pick up date, a certified letter will be sent regarding the animal's abandonment. If the pet is not claimed and fees paid within the specified time set forth in the letter, the client will relinquish all rights to this abandoned animal. The animal in question will become the property of Hickory Pet Inn/Hickory Veterinary Hospital.

Kennel Cough Informed Consent: We make every effort to ensure our facility is free of diseases including Canine Infectious Tracheobronchitis (Kennel Cough) and Canine Influenza (CIV). Despite our best efforts, we cannot guarantee that your pet will not be exposed to a strain not covered by the current vaccine. If your pet presents with signs of an upper respiratory infection, please call us as soon as possible.

Flea and Tick preventative is encouraged but not required to board. If your pet has fleas, Capstar (OTC oral preventative) may be provided at your cost.

Vaccines Required: Daycare, playcare and boarded animals must be current on all vaccines and free of parasites. By writing your name below, you agree that if you cannot provide written proof, you authorize the doctor to provide vaccines and parasite control as needed.
